Primary Care Physician Opportunity - Outpatient Community Health

Location: Charlestown, NH - Brattleboro 35m

Employment Type: Full-time, permanent position

Position Type: Employed, Federally Qualified Health Center (FQHC). Position Overview. Seeking a Primary Care Physician to join a collaborative community health team serving patients in southern New Hampshire and nearby Vermont communities. This role focuses on providing comprehensive outpatient care in a patient-centered environment with integrated behavioral health services and strong care coordination support. The position is ideal for physicians who value team-based care, evidence-based medicine, and developing long-term relationships with patients while managing a diverse and sometimes complex patient population.

Qualifications / Candidate Requirements

  • MD or DO from an accredited medical school
  • Board Certified or Board Eligible in Family Medicine or Internal Medicine
  • Completion of a 3-year residency training program
  • Eligible for medical licensure in New Hampshire
  • Strong clinical knowledge of common primary care conditions
  • Excellent communication and patient engagement skills
  • Commitment to evidence-based medicine and collaborative care
