Your Impact & Responsibilities:

  • Comprehensive Evaluations: Perform high-quality physical therapy evaluations, developing personalized treatment plans for post-acute and long-term care residents.
  • Skilled Interventions: Implement evidence-based treatments to improve resident mobility, balance, strength, and overall functional independence.
  • IDT Collaboration: Partner closely with nursing, therapy, and medical providers to ensure a holistic approach to resident recovery.
  • Expert Documentation: Maintain precise and timely documentation in accordance with California state and federal regulatory requirements.

Qualifications:

  • Licensure: Current, valid California Physical Therapist (PT) license in good standing.
  • Skills: Strong communication, clinical reasoning, and a passion for serving the geriatric and post-acute populations.
  • Experience: Previous SNF experience is a plus, but New Graduates are encouraged to apply!
