Your Impact & Responsibilities:

  • Comprehensive Evaluations: Complete initial assessments and develop individualized treatment plans for residents with functional, cognitive, or ADL limitations.
  • Therapeutic Intervention: Provide skilled treatments targeting ADLs, functional mobility, strength, cognition, and safety.
  • Equipment & Innovation: Recommend adaptive equipment and environmental modifications to ensure a safe transition back to home life.
  • Interdisciplinary Collaboration: Work closely with PT, SLP, Nursing, and Medical teams to deliver a holistic, coordinated approach to recovery.
  • Expert Documentation: Maintain precise, timely documentation of resident progress in accordance with California state standards.

Qualifications:

  • Licensure: Current, valid California Occupational Therapist (OT) license in good standing.
  • Skills: Strong communication, clinical reasoning, and a passion for the geriatric population.
  • Experience: SNF experience is preferred, but New Graduates with strong clinical rotations are encouraged to apply!
