Job description

Role Overview: A licensed Physical Therapist provides personalized, one-on-one care to older adults in private residences and senior living communities, supporting mobility, independence, and quality of life through evidence-based treatments.

Responsibilities Include

  • Conduct evaluations and develop individualized care plans
  • Deliver therapy sessions to promote wellness and functional independence
  • Document patient progress accurately within electronic medical records
  • Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists
  • Educate patients, caregivers, and staff on safety and aging in place
  • Utilize telehealth and virtual care to enhance access and flexibility

Qualifications And Skills

  • Valid Physical Therapy license or eligibility in practice state(s)
  • Degree from an accredited PT program
  • Proficiency in basic computer and EMR systems
  • Current CPR certification

Physical Requirements: Ability to perform moderate physical efforts, assist with patient transfers, reposition patients, and drive between patient locations. Must meet health and safety standards, including PPE use when required.
