Job description

Role: Licensed Physical Therapist delivering personalized, one-on-one care to older adults in private residences and senior living communities.

Responsibilities: Conduct evaluations, develop individualized treatment plans, and provide evidence-based therapy sessions to enhance mobility, independence, and overall well-being. Document patient progress accurately in EMR systems, collaborate with interdisciplinary teams, and educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to increase access and flexibility.

Qualifications & Skills: Valid state license or eligibility, degree from an accredited PT program, proficiency in computer and EMR use, and current CPR certification. Physical stamina to perform transfers, repositioning, and extended walking, as well as the ability to exert moderate physical effort, including lifting and assisting with patient mobility.
