Job description
Role: Physical Therapist
Responsibilities:
- Provide one-on-one physical therapy to older adults in private homes and senior living communities
- Conduct evaluations, develop personalized care plans, and deliver evidence-based treatments
- Promote patient wellness, mobility, and independence
- Document care within electronic medical records in compliance with regulations
- Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging in place
- Utilize telehealth and virtual care to enhance access and flexibility
Qualifications & Skills:
- Valid PT license or eligibility in the practice state
- Degree from an accredited PT program
- Proficiency in computer use and EMR systems
- Current CPR certification
Physical & Other Requirements:
- Ability to perform moderate physical effort, including lifting and repositioning patients
- Capable of driving to patient locations
- Willingness to wear PPE when necessary
