What You’ll Do:
- Nutrition assessments and patient care planning.
- Monitor weight changes, nutritional status, wounds, and nutrition-related concerns.
- Provide recommendations for therapeutic diets, supplements, and tube feeding when appropriate.
- Monitor weight changes, nutritional status, wounds, and nutrition-related concerns.
- Educate patients regarding nutrition to promote wellness.
- Collaborate with Physicians, Nurses, Speech & Occupational Therapists, and the interdisciplinary care team.
- Help improve patient outcomes using “Food as Medicine”.
