Location: Fairfield, CA - Hybrid. This is a field-based home health position serving patients throughout Solano County, with documentation completed from the field.

Note: Candidates must have an active California Occupational Therapist (OT) license and 1+ year of Occupational Therapist experience. Home health experience, an accredited Occupational Therapy degree, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.

Job Description

  • Evaluate patients' functional abilities in their home environments and develop personalized treatment plans.
  • Help patients improve their ability to complete daily living activities such as dressing, bathing, cooking, and household tasks.
  • Implement therapeutic activities designed to improve independence, safety, coordination, and functional performance.
  • Recommend adaptive equipment and practical strategies that make everyday activities easier and safer.
  • Educate patients and caregivers on techniques that support continued progress and independence.
  • Monitor patient outcomes and update treatment plans as needs change.
  • Collaborate with nurses, physicians, therapists, caregivers, and other members of the home health team.
  • Complete accurate and timely clinical documentation.
  • Travel throughout Solano County to provide personalized in-home occupational therapy services.

Qualifications

  • Active California Occupational Therapist (OT) license required.
  • 1+ year of Occupational Therapist experience required.
  • Home health Occupational Therapist experience preferred.
  • Graduate of an accredited Occupational Therapy program preferred.
  • Current BLS or hands-on CPR certification preferred.
  • Valid driver's license and reliable insured vehicle preferred.
  • HCHB/Homecare Homebase experience preferred.
  • OASIS experience preferred.
