Job description
HIRING: AI PRODUCT ENGINEER — CLINICAL AI
We’re looking for an AI Product Engineer to join a fast-moving healthcare AI team building the technical foundation of an agentic clinical AI platform.
Candidates with a strong ML research background but without hands-on product engineering experience are not a fit for this role.
This role is ideal for a product-minded full-stack engineer who enjoys working across applied AI, backend systems, LLMs, and user-facing product experiences and wants to build AI systems that solve complex, real-world clinical workflows.
Location: New York City, NY
Work Model: Hybrid
Employment Type: Full-time
Salary: $220K – $245K + Competitive Equity
Experience: 3+ years in product engineering, with a focus on LLM-based systems
VISA: Visa sponsorship/ Open to visa transfers (H1B transfers)
WHAT YOU’LL DO
- Build and evolve an agentic clinical AI platform, including context management, tool use, orchestration, permissions, memory, and human-in-the-loop workflows.
- Rapidly prototype new clinical AI experiences and transform promising concepts into production-quality software used by real clinicians.
- Design AI evaluations, regression suites, and launch criteria to measure task success, clinical quality, reliability, safety, latency, and cost.
- Build robust observability infrastructure across model behavior, tool calls, traces, errors, and end-to-end workflow outcomes.
- Collaborate closely with clinicians, product leaders, and engineers to translate complex clinical workflows into reliable and trustworthy software.
- Own meaningful product capabilities from early prototype through production, helping shape the architecture and user experience of the platform.
REQUIRED TECHNICAL SKILLS
- Python
- TypeScript
- LLMs / Large Language Models
- Agentic AI frameworks
- RAG / Retrieval Systems
- APIs & Backend Services
- Production experience with tool calling, orchestration, and context management
- Experience building LLM evaluations, regression testing, or AI observability tooling
REQUIRED EXPERIENCE
- 3+ years of product engineering experience with a focus on LLM-based systems
- Proven experience building agentic or LLM systems in production
- Experience working within healthcare or another highly regulated domain
- Experience conceptualizing, architecting, and building a product end-to-end
- Strong understanding of AI system reliability, evaluation, observability, and production deployment
- Ability to communicate technical trade-offs clearly to non-technical stakeholders.
NICE TO HAVE
Experience with:
- Agent harnesses
- Workflow engines
- Coding-agent infrastructure
- Advanced agent orchestration systems
- AI evaluation and observability infrastructure
WHO WE’RE LOOKING FOR
We’re looking for someone who is more than a pure ML researcher — someone who can take an ambiguous problem, design the solution, build the product, evaluate it, and take it into production.
The ideal candidate combines:
- Applied AI expertise
- Strong product engineering skills
- Backend and full-stack development
- Hands-on LLM/agentic AI experience
- Understanding of healthcare or regulated environments
- Strong evaluation and observability mindset
- Excellent communication and collaboration skills
INTERESTED?
If you have hands-on experience building LLM-powered products, agentic systems, AI workflows, or production AI infrastructure and are excited about applying these technologies to healthcare, we’d love to hear from you.

Visa sponsorship record
No public sponsorship records foundWe didn't find H-1B, H-1B1, E-3 or green card (PERM) filings under the name Carnaby Fox in the Department of Labor and USCIS data. That doesn't mean the job can't be sponsored — the company may file under a different legal name, be new to sponsorship, or sponsor a visa that isn't in these datasets (for example H-2B or J-1).
Tip: ask the recruiter early, in writing, which visa they sponsor and whether they cover the legal and filing fees.

Occupation: Software Developers (SOC 15-1252).
- Mentioned in the listing
H-1B
- H-1B cap-exempt employer
Regular cap-subject employer — a new H-1B needs to win the lottery in March (unless you already hold cap-counted H-1B status).
- TN (citizens of Canada and Mexico)
This kind of role may fit the USMCA profession “Computer Systems Analyst (or Engineer, for engineering-degree holders)” — it depends on the actual duties. No lottery, no cap; you need the matching degree or license.
- E-3 (Australia) and H-1B1 (Chile, Singapore)
Same degree requirement as H-1B, but no lottery and a separate quota that is rarely filled.
- O-1 (extraordinary ability)
For candidates with awards, publications, press, a high salary or critical roles at distinguished organizations. No cap, no lottery; the employer files a petition.
- STEM OPT extension (F-1 students)
Requires an E-Verify employer. We didn't find this company in the E-Verify list — ask HR.
Salary vs prevailing wage
Level IV (fully competent)Software Developers · New York-Newark-Jersey City, NY-NJ. Annual prevailing wages set by the Department of Labor (OFLC).
| Level | Prevailing wage | H-1B lottery odds* |
|---|---|---|
| Level I | $109,845 | ~15% |
| Level II | $137,717 | ~31% |
| Level III | $165,589 | ~46% |
| Level IV | $193,461 | ~61% |
This job pays $220,000–$245,000 a year — that's Level IV (fully competent). In the wage-weighted H-1B lottery a Level IV registration gets 4 entries; estimated selection chance about 61% — better than average.
* Odds are DHS projections for the FY2027 wage-weighted lottery (actual results vary by year and employer). Since the FY2027 cap season the H-1B lottery is weighted by wage level: Level I = 1 entry, II = 2, III = 3, IV = 4. The level is set by the offered wage against the prevailing wage for the occupation and worksite. Separately, a $100,000 fee for new H-1B petitions for workers outside the US was announced in 2025; as of September 2026 a federal court ruling keeps it unenforceable while appeals continue — check the current status.
